Parents or legal guardians – ALL DISTRICTS
You are required to submit the Student Information & Permission form two weeks before your scheduled Outdoor School program for your child to participate. The Coyote Outdoor School required forms are linked in orange buttons above and embedded below in English & Spanish. Your school district may require additional forms and health information.
